We use a simple, structured process to understand what support is needed and how it should be delivered. The exact process may vary depending on urgency, complexity and the type of care required.
Tell us about the person, the support being considered, preferred times, current challenges and any immediate priorities.
Where the enquiry is suitable, we gather more detailed information about needs, preferences, routines, risks, communication, mobility, medication and desired outcomes.
We document how support should be provided, what matters to the person, identified risks, agreed tasks and key information staff need to know.
We organise the care team and practical arrangements, taking account of required skills, compatibility and continuity where possible.
Support starts in line with the agreed plan. Staff record relevant information and communicate concerns through the correct channels.
Care should evolve when needs change. We review feedback, incidents, concerns and changes in circumstances and update arrangements where required.
Where the person has agreed, or where there is another lawful basis for involvement, we work constructively with relatives and representatives. Clear boundaries around consent, confidentiality, decision-making and information sharing are maintained.
Tell us what support you’re considering and we’ll guide you through the process.
